Through Tracks, which is about promoting wellbeing, we support and care for young people who are at risk of developing a serious mental illness.
Tracks is a support program for young people aged 12 to 25 and their families. It aims to improve the wellbeing and mental health of young people by providing youth work, case management and shared-cared response from a range of services. We work together to care for and support the young person and their family.
We work with those at risk of developing a serious mental illness like psychosis, depression or anxiety, providing holistic support that targets factors that are impacting young people and their family’s wellbeing.
Tracks offers mentoring, relationships and information to enhance young people’s health and wellbeing; as well as advocacy, referrals and support to access specialist services.
