The Reconnect is for young people aged 12 to 18 years who are at risk of homelessness or are homeless. The RECONNECT Youth Workers assist young people to improve their lives by providing mentoring, advocacy, information, family mediation and reconnection, and referrals to relevant specialist services. This service is available in Albany, Denmark and Mt Barker.
